Allied Health referral and intake process

All new referrals are sent to our clinical leader to review. Referrals are carefully reviewed and allocated to clinicians that would be considered the best fit for the client.

If there is a waiting list, the coordinator/referrer will be advised of the approx. waiting time. If there is no waitlist, the client will be allocated and the clinician will make contact within 48hrs following allocation.

The clinician will contact the client to introduce themselves and arrange an initial meeting.

The initial meeting will generally occur at one of our clinic locations. However, if this is not suitable other arrangements can be organised. Please note travel costs apply for outside visits in line with the NDIS support guide.

The initial meeting involves the following.

Clients and/or family will have an opportunity to meet the clinician, ask questions and sign relevant forms. If the client is a young child, we are more than happy for them to attend the initial meeting; however, please note we may discuss sensitive information pertaining to your child’s behaviour. We are happy to organise a follow-up telephone call if needed for parents to provide this confidential information.

  • If you have any copies of previous reports, assessment reports or behaviour support plans, we encourage you to send them through with the referral or bring them to the initial meeting for the clinician to review.
  • The clinician will seek relevant background information and details of current concerns.
  • The clinician will also ask about the clients/families goals for the intervention and develop an intervention/timeline of how the funding will be used and how the intervention will be delivered.

We understand the NDIS can be confusing and frustrating at times, so we strive to ensure your experience with us is a positive one. If you would like to make a referral to our service please call us on 02 4013 6079, complete the manual referral form here, or submit the form below.
